Job Overview:

This position has responsibility for the effective implementation of the philosophy, goals, policies, and procedures of the organization and the department and their effect on daily operations. This role has authority to make decisions in the department in the areas of operations or patient care as applicable, human resource, and financial management.

Job Requirements:

Bachelor's Degree
Basic Life Support for Healthcare Providers (BLS)
Physical Therapist
Occupational Therapist
5-7 years experience Professional

Job Responsibilities:

Provides comprehensive and appropriate treatment, including periodic re-assessment and goal modification, patient/family education, on-going discharge planning and communication within the team.
Meets established departmental productivity standards of Visits/Procedures and coverage of call patients as needed.
Comprehensively evaluates and develops functional goal driven treatment plans considering the patient?s age, educational level, family support, and cultural and psychosocial issues.
Completes all written documentation in accordance with departmental policy and procedure for timeliness, format, and content.
Collaborates with the PTA / ATC to ensure that the established treatment plan is carried out, that the patient is progressing toward discharge, and that periodic reassessment and treatment modification occurs when indicated by patient status.
Accurately completes specific departmental delegated duties in a timely manner.

Leadership Tactics:
Conduct department huddles. Generally, clinical departments hold daily huddles, non-clinical hold weekly huddles.
Regularly Round on Team Members, using questions from the rounding log.
- 25 or fewer team members = monthly
- 26-50 team members = every other month
- 51+ (and optional team members) = quarterly

Lead monthly team meetings using meeting agenda template; review stoplight report; cascade key leadership messages.

Model, coach and validate team members' use of TriHealth Way behaviors (AIDET + Promise, Always Behaviors and Always HEARD).
