Benchmark's Supported Living program assists adults with daily activity skills to gain independence in small personal home settings. Benchmark is seeking an LPN Healthcare Coordinator to provide healthcare coordination and direct intervention, as needed, for designated individuals in the Supportive Living and Adult Day Services. The nurse will also provide training for unlicensed staff to meet the healthcare needs of individuals served.
Part Time!! 25-30 hrs/week - flexible schedule and mileage reimbursement!!
